I am writing to request a review of the EHC plan for {child_name}. The
plan was last reviewed on {last_review_date}.

Legal basis for this request

Under s.44(1) of the Children and Families Act 2014 and Regulation 18
of the SEND Regulations 2014, the Local Authority must review every
EHC plan at least every 12 months.

I am also relying on Regulation 20 of the SEND Regulations 2014, which
requires the Local Authority to review a plan where there has been a
significant change of circumstances. The significant change(s) of
circumstance for {child_name} are:

Please confirm the date of the review meeting in writing. So that I
can take part meaningfully I am asking for:

  - copies of all advice, reports and paperwork to be circulated to
    everyone attending at least 2 weeks before the meeting (SEND Code
    of Practice 9.176);
  - written notice from the LA within 10 working days of the meeting
    setting out whether it intends to amend, maintain or cease the
    plan, with reasons (SEND Code of Practice 9.176-9.184).

Attendees I would like invited

I am exercising my right under the SEND Code of Practice (9.176) to
ask that the following attendees, in addition to the school and case
officer, are invited to the review:

  - the educational psychologist who knows {child_name}
  - any therapists currently providing or recommending provision for
    {child_name} (e.g. speech and language, occupational therapy)
  - the social worker (where social care is involved)

Please confirm in writing whether they will attend.
