Use this letter to ask a professional, school or clinic to put what they know about your child in writing — for example to confirm how much support they actually provide, or to send you a copy of a report they hold.

Dear {recipient_name},

Re: {child_name}, DoB {child_dob} — request for information in writing

I am {child_name}'s parent. I am gathering evidence about {child_name}'s
special educational needs, and I would be grateful if you could provide
the following in writing:

{evidence_requested}

Why I am asking
---------------
{why_needed}

What would help most
--------------------
Please describe what you have seen yourself and what you would
recommend. Where you are able to say so, it helps a great deal if you
can set out how much support is needed, how often it should happen, and
who should deliver it. Provision described at that level of detail is
far more useful than a general statement of need, because the LA is
expected to specify support in Section F of the EHCP clearly enough
that everyone knows what must be delivered.

If you hold any reports or records about {child_name} that you have not
already shared with me, please treat this letter as a request for copies
of them. I am happy to confirm my identity and to pay any reasonable
copying cost.

Please reply by {reply_by_date} if you can. If that is not possible, I
would be grateful if you could let me know when I can expect to hear
from you.

Thank you for your help.

Yours sincerely,

{parent_name}
{address}
