Diary of concerns and incidents

Keep dated notes of specific examples where your child struggles. Include what happened, how your child reacted, and what support was given.

School records and reports

Request copies of school reports, IEPs, SEN Support plans, and any assessments the school has carried out.

Professional reports (GP, paediatrician, therapists)

Gather any reports from health professionals, speech and language therapists, occupational therapists, CAMHS, etc.

Examples of your child's school work

Keep examples showing the level your child is working at compared to age expectations.

Correspondence with school about concerns

Save all emails, letters, and notes from meetings with the school about your child's needs.
